Because Cortex creates medico-legal documents, there are some restrictions around editing or deleting notes.
The following rules apply
- Any Clinical Note that is in draft can be deleted by the Note Owner. This will remove the Note from the patient timeline and it will no longer be available for viewing.
- Any signed note is a formal part of the clinical record. It will be published to your organisations clinical document repository and will be visible within the Cortex web view tab.
Addending a Note
A signed note can be addended. So in practice, if there is a minor error, you can Tap the action menu button (bottom right) in the Note and document an addendum. The addendum will be added to the note in Cortex and published. Notes with addendums will continue to sit within the Cortex timeline at the time the Note was initially signed, not when the addendum was added.
- Please consider informing members of the the clinical team about the addendum if relevant to them.
Rescinding a Note
If there is a fundamental error in the note that cannot be easily addressed with an addendum, there is the option to rescind a note.
This option is also available via the action menu button (bottom right). You will have to provide a reason for rescinding the note.
Once rescinded, the note details are no longer viewable in Cortex although the Note event remains in the patient timeline. There is a 'break glass' function so details can be viewed, but a reason for viewing will need to be provided. The note is also removed from any published area outside of Cortex.
